Complete and return to our main office via e-mail, fax, or in person. DO NOTgive to Sales or Field personnel. You will be contacted by the Warranty Services Department who will arrange for inspection and repair of valid warranty items. You or someone at least 18 years of age must be present in your home at all times while inspections or repairs are being made.
